Tuesday June 6, Sports Physicals will be conducted at the Middle/High School.

This is a yearly, free service provided by Dr Bream, our school physician.This is not intended to take the place of an annual physical with their Primary Care Provider. It will allow students to participate in school sports thru the 2017-2018 school year.

 All students are required to have a parent/guardian complete and sign the permission form to submit to the school nurse on or before June 6.

Students may not have a physical without a complete and signed parent permission form.

The permission forms are available in nurse’s office, or in the main office of the middle/high school.

 On June 6, students are to wait in the cafeteria at the end of the school day at 2:05 pm. It is on a first come, first served basis.

The physicals are conducted by Dr. Bream, in the nurse’s office, with the school nurse nearby.
