Sports physicals with Dr. Bream will be held Tuesday June 3 at 2:05 pm in the cafeteria, first-come, first-served.

Parents are asked to fill out the attached medical history form to provide to Dr. Bream.

Please complete the medical history and send it with student on June 3 or deliver it to the school nurse prior to June 3.

Students entering grades 7 and 10 are required to have a complete physical by their own physician anytime from March 2014 to March 2015. This complete physical with a full list of vaccinations must be provided to the school nurse.

Students entering grade 7 must show proof of a tetanus booster and second varicella (chicken pox) vaccine when they provide their physical to the nurse.

Medications in the nurse’s office must be picked up by a parent at the end of the school year. If not picked up, they will be disposed of properly.

Medication orders, consent forms, and medications must be renewed at the beginning of every school year.  Forms may be picked up in the nurse’s office or the main office.

Doctor’s may fax the orders directly to the nurse’s secure fax at 508-835-6075.
